I have read and understood the conditions of entry and completion requirements of my selected course here on the College website.

Where I am not accessing VET Student Loans to pay for my course, or if a cash co-payment is required to pay the balance of my tuition fees I do have the financial capability to meet my tuition fees and agree to pay them as they become due. The information provided in this application form and any accompanying documents is correct.

I will inform the College if my contact details change (including mail, email and/or phone).

I authorise the College to verify the authenticity of my academic/professional qualifications and my work experience and I understand the College may inform other organisations or regulatory agencies if any of the information in my application is not accurate.If under 18 a copy of your letter of offer must be signed by a legal guardian or parent, in accordance with the terms and conditions of this agreement prior to your enrolment being finalised. If signed by a legal guardian or parent they guarantee payment of all financial obligations of the student.

I hereby consent to the collection and use of my personal images by photography or video recording. I acknowledge these may be used on the Ella Bache College website, social media and advertising.

I also understand that my consent can be withdrawn at any time in writing to studentservices@ellabachecollege.edu.au

I agree to remove artificial nails and enhancements, including acrylic, gel, shellac (hands and feet), prior to commencing the course. I understand that I cannot perform treatments or be assessed with artificial enhancements.I agree to remove lash extensions prior to commencing the course. I understand that I will not be able to participate in the Provide Lash and Brow unit with lash extensions.
